Primera cohorte comunitaria post-COVID-19 en el norte del Perú: protocolo para la evaluación bimodal de las secuelas pulmonares y psicológicas. Estudio SUYAY TUMPIS.

La pandemia por SARS-CoV-2 generó una carga significativa de secuelas post-COVID-19, que incluyen deterioro de la función pulmonar y morbilidad psicológica. En el Perú, la evidencia a nivel comunitario sobre estas secuelas es escasa, particularmente en entornos rurales. SUYAY TUMPIS es un estudio observacional de cohorte abierta ambidireccional diseñado para determinar las secuelas pulmonares y psicológicas en supervivientes de COVID-19 procedentes de comunidades urbanas y rurales de la región Tumbes, norte del Perú. Se enrolará a un total de 270 adultos (edad ≥18 años) con antecedente de COVID-19 entre 2020 y 2021, procedentes de Puerto Pizarro (rural) y el AAHH Las Flores (periurbano), junto con 30 controles negativos para COVID-19. Las evaluaciones se realizarán en los meses 0, 6 (condicional) y 12. La función pulmonar se evaluará mediante espirometría portátil (Datospir Aira-T: FVC, FEV1, FEV1/FVC, PEF) y tomografía computada de tórax sin contraste (SIEMENS SOMATOM GO, 16 cortes) al inicio del estudio. La ansiedad se tamizará mediante el cuestionario de Trastorno de Ansiedad Generalizada-7 (GAD-7), aplicado durante visitas domiciliarias en los meses 0 y 12. El análisis estadístico incluirá modelos lineales generalizados (familia Poisson), análisis de sobrevida (Kaplan-Meier, regresión de Cox) y ANOVA de medidas repetidas. SUYAY TUMPIS es la primera cohorte comunitaria post-COVID-19 en el norte del Perú que incorpora evaluación bimodal (pulmonar y psicológica). Sus resultados contribuirán a la planificación sanitaria regional basada en evidencia.
