Diagnóstico y tratamiento de un segundo premolar mandibular con tres conductos radiculares: reporte de caso

Introducción: La omisión de conductos radiculares es una causa frecuente de fracaso en el tratamiento endodóntico. Se han registrado variaciones anatómicas complejas en los premolares mandibulares, que pueden incrementar la dificultad del tratamiento y comprometer su pronóstico. Reporte de caso: Se describe el diagnóstico y manejo de un segundo premolar mandibular con tres conductos radiculares y forámenes independientes. El diagnóstico incluyó el análisis de radiografías periapicales con distintas angulaciones y una tomografía computarizada de haz cónico. El tratamiento incluyó un acceso cameral modificado, instrumentación reciprocante, irrigación activa y obturación con técnica de onda continua de calor. Conclusiones: La identificación de variaciones anatómicas mediante criterios diagnósticos basados en evidencia y herramientas imagenológicas complementarias optimizan el tratamiento endodóntico y reducen el riesgo de fracaso terapéutico.
